What IS causing the NHS crisis?

A friend was supposed to get a call from a GP at 6 and they still haven't called. How can it take them longer than it did in the surgery when there's no physical interaction?

Why are ambulances still backed up (why can't they lay on something else for non emergency transport??)?

It doesn't seem like just one thing, it seems like everyone has given up at once.

Part of the deal with doctors when the NHS was set up is that they would get some time to do private work.  The problem is that some exploit this to the full and do the minimum contractual requirement for the NHS while others work 60-70 hours plus for the NHS without extra pay.  It depends on your motivations I guess.

I'd be interested to know how many lawyers are paid differently for X as opposed to X+20%. I would have retired already. 

But yes, it sounds as though the consultants' contract is pretty shit for the taxpayer. Bit like the GP contract really. Slow handclap for the government negotiators in both cases. 

that +10% before you can run a private clinic is interesting - I didn't know that. Explains why so many doctors round here have quit their private clinics and set up botox / fillers side gigs (ie unregulated, no need to give the NHS more time, but £300 for 15 minutes)

Is it even a 40 hour week?
 

Yes, 10-11 PAs should be around a 40 hour week. The exception is that time spent working out of hours is weighted more heavily. I think a (rare but increasingly more common) consultant resident night shift counts as enough PAs for 2 normal days, though this depends somewhat on local arrangements.

As DDS says, arguments about whether consultants “should” be working harder for the NHS are neither here nor there unless there is some way to make this happen while still retaining the incredibly stretched goodwill of the workforce. That’s why I say whether or not the current contract is sufficiently demanding isn’t really the point - realistically I don’t think asking for more effort from consultants is going to achieve the desired effect at present.

That’s just on paper of course. Suspect many consultants work well over their contracted PA time. I know one of my gastroenterologist bosses would usually be leaving halfway through my evening on-call shifts so around 7-8pm.
